Qualified Intellectual Disability Professional (QIDP)

Overview: A Full-Time (40 Hours/Week) Qualified Intellectual Disability Professional, providing Service Facilitation/Self Directed Assistance in the Home-Based Support Services program through the Department of Human Services. Essential characteristics of self-motivation, flexibility and a concierge outlook to provide the highest quality services for our clients.
Description/Job Duties: Manage a caseload of both children and adults with intellectual disabilities in the home-based program. Collaborate with client’s support team members and facilitate team meetings. Construction of the implementation strategy plan, ongoing monitoring and implementation of services to ensure consistency and continuity of care. Conduct assessments, create and identify needs, conduct research and coordinate services. A considerable amount of advocacy, documentation and communication with clients, families and providers. Maintain client files in accordance with IDHS DD and agency requirements. Maximize available billable hours and complete and submit billing documentation on time. Other duties as assigned.
Qualifications:
• Bachelor's Degree in a human services-related program (Required)
• Minimum of one year experience working with children and/or adults with developmental disabilities (Required)
• Willingness to travel often and work in client's home environment (Required).
• Valid Illinois Drivers License (Required)
• Current certification and experience as a Qualified Intellectual Disability Professional (Required)
• Experience working with the Department of Human Services (Preferred)


Full-Time Day Program Coordinator/Direct Support Person (DSP)

Overview: A Day Program Coordinator is a Direct Support Professional and provides oversight of adult clients in a
day program setting. Creates and implements activities, outings, programs, etc. Responsible for safe
transportation of clients in company vehicle. Essential characteristics of self-motivation, flexibility and a
concierge outlook to provide the highest quality services for our clients
Description/Job Duties: Provide direct support to adults with intellectual disabilities in PremierCare’s day
program, MyDay. Collaborate with client support team members and ensure that individuals exercise their rights
and are treated with dignity and respect. Responsible for the implementation and documentation of individual
goals, objectives, behavior tracking and interventions when applicable. Assist individuals with their activities of
daily living (ADL), promoting independence including, but not limited to, daily tasks, recreational activities,
physical fitness activities, grooming and dressing, money management and community integration. Know the
whereabouts of individuals you are directly responsible for at all times. Attend training as required to continue
skill development as necessary. Respond to all medical or behavioral concerns and accidents / incidents
immediately per policy and procedures. Other duties assigned.
Qualifications:
• High school diploma or GED
• Must be 18 years of age
• Valid driver’s license in good standing required
• Effective communication skills
• Current certified Direct Support Person (DSP) or completion of DSP certification upon hire including CPR and
First Aid Certification
• Experience and understanding of individuals with a developmental disabilities or mental health diagnoses
• Successful clearance of local and/or state background checks


Personal Support Worker (PSW)/Direct Support Person (DSP)

Overview: A Personal Support Worker/Direct Support Person to provide direct support to children and adults with
developmental disabilities in the Home-Based Support Services program through the Department of Human
Services. Essential characteristics of self-motivation, flexibility and a concierge outlook to provide the highest
quality services for our clients
Description/Job Duties: Provide direct support to both children and adults with intellectual disabilities in the
home-based program. Collaborate with client support team members and ensure that individuals exercise their
rights and are treated with dignity and respect. Responsible for the implementation and documentation of
individual goals, objectives, behavior tracking and interventions when applicable in the Individual Service Plan
and behavior plan of each individual served. Assist individuals with their activities of daily living (ADL), promoting
independence including, but not limited to, household chores, recreational activities, physical fitness activities,
grooming and dressing, money management and community integration. Know the whereabouts of individuals you
are directly responsible for at all times. Attend training as required to continue skill development as necessary.
Meet the needs of the individual served and their family/guardian(s). Report medical or behavioral concerns and
accidents / incidents immediately to parent/guardian and program director. Report any and all allegations of
individual abuse or neglect that is reported to or witnessed immediately to the Program Director and Office of
Inspector General (OIG). Other duties assigned.
Hours: Variable schedule, on an at need basis. Daily schedule flexibility is required, including afternoon and
evening availability.
Qualifications:
• High school diploma or GED
• Must be 18 years of age
• Valid driver’s license in good standing required
• Access to a registered vehicle with proof of insurance to provide transportation for individuals served
• Ability to work with both children and adults
• Good communication skills
• Current certified Direct Support Person (DSP) or completion of DSP certification upon hire
• Experience and understanding of individuals with a developmental disabilities or mental health diagnoses
• Successful clearance of local and/or state background checks


Behavior Therapist

Overview: A Behavior Therapist to provide home-based services for adults and children with intellectual and
developmental disabilities. The Behavior Therapist is responsible for assessment, evaluation, creation and implementation of Behavior Intervention Plans, accurate and reliable data collection, consultation and training with family and personal support workers. The BCBA will collaborate with client’s support team members and attend team meetings. Ongoing monitoring and implementation of behavior plan to ensure all behavior and skill
acquisition needs are addressed.
Description/Job Duties: Monitors progress on at least a monthly basis and more frequently if needed to address issues with the participant's outcomes. Revises the behavior strategies as needed to ensure efficacy. Prepares
progress reports to the service planning team at determined intervals. Supervises implementation of the
behavior plan, including training of the personal support staff, unpaid informal care givers and family to
ensure they properly apply the interventions. Understand the specific services and outcomes for the
participant being served. Maintain regular communication with support team and provide timely reports
and progress updates regarding client’s progress. Meet monthly billable minimums. Other duties as
assigned.
Requirements:
• Bachelor’s Degree in a human services-related program. Master’s Degree Preferred.
• Meet the qualifications of a Level 2 Behavior Therapists with DHS-DDD.
• Minimum of 1 year experience working with children and/or adults with developmental disabilities.
• Willingness to travel often and work in client’s home environment.
• Valid Illinois Drivers License and auto insurance.
